Below are the Health Forms that Holy Infant requires all students to have. If you child has
asthma, food allergies, or takes medication during school hours, these mandatory forms must be completed by your child's health care
provider regardless of grade level. If you have any questions, contact the
School Nurse.

From the Nurse..
As of December 15th there have been 12 positive and 10 highly probable cases of Pertussis, also know as whooping
cough, at Holy Infant School. Incubation can be as long as 3 weeks after exposure so it is possible that children will begin
to show symptoms over Christmas Break. Your child might have cold symptoms that develop into persistent coughing; cough that
disturbs sleep; fits of coughing that may lead to vomiting; difficulty catching their breath after coughing. The characteristic
whoop sound to gasp in air after a coughing fit may or may not be present. See your doctor and inform them of the situation
here at school. Treatment is a course of effective antibiotics which your doctor can prescribe with or without the nasal swab
for lab testing. After 5 days or antibiotic treatment the Pertussis is no longer contagious; children should be quarantined
for those 5 days.
