Health Forms
Below are the Health Forms that Holy Infant requires all to students to have on file. If your child has asthma, food allergies, or takes medication during school hours, these Mandatory forms must be completed by your child's health care provider regardless of grade level. Physical Exam Form | required for all incoming Pre-School, Jr-Kindergarten, Kindergarten, 3rd grade, 6th grade and all new students.
Food Allergy Action Plan | required for all students who have food allergies
Parental Consent/Medication Administration (As Needed) | required for all students taking medication at school.
